   [Payer] CMS final TAVR NCD (CAG-00430R2) Decision Memo released 10 Sep 2026. Updates: (1) symptomatic severe AS covered without CED; (2) asymptomatic severe AS covered under CED; (3) revised heart-team / operator / hospital criteria. Device must have FDA PMA for the indicated use. Operator volume: ≥20 total transcatheter cardiac valve (≥15 TAVR) per year or ≥40 (≥30 TAVR) every 2 years. Joint dual-operator participation not required. Hospital: on-site structural IC + cardiac surgery; ICU experienced with open-heart valve patients; continuous QI. Asymptomatic CED studies need CMS approval + contemporaneous comparator.
